Project Description
The system for monitoring the outcomes of outpatient treatment with an emphasis on chronic patients in public pharmacies (abbreviated as PILK) has been implemented as part of the National Recovery and Resilience Plan 2021–2026 of the Government of the Republic of Croatia (reference number: C5.1.R4-I5), in collaboration with the Croatian Chamber of Pharmacists.
PILK enables the comprehensive and transparent monitoring of treatment outcomes for several prevalent chronic diseases, including diabetes, hypertension, asthma and others. The system also supports the recording of patient pharmacological history, monitoring of patient adherence to medicine treatments, identification of medicine prescription errors, and assistance in prescribing antibiotic therapy. Thereby, the system facilitates the creation of valuable pharmacoepidemiological databases, paving the way for more systematic planning, monitoring, and evaluation of the cost-effectiveness of investments in pharmacotherapy for outpatients.
Additionally, PILK provides support for patient education through several protocols, including introduction to a new medicine therapy and counselling on national preventive programmes.
The PILK system is integrated with the web services of the eMedicines system, which governs the national medicine database, allowing access to necessary medicine data, including medicine interactions and contraindications. It is also connected to HALMED OPeN for reporting medicine adverse reactions, and to several services of the Croatian Health Insurance Fund (HZZO) for access to realized prescription data and administrative data on patients, medical doctors and pharmacists.
The PILK system also enables the sending of structured documents according to FHIR communication messages to the CEZIH system, including communication with selected medical doctors.
The system is accessible to pharmacists in Croatian public pharmacies, as well as pharmaceutical technicians with appropriate authorization rights. Data entered in a particular pharmacy can be accessed with read-only permissions in other Croatian pharmacies that dispense prescription medicines, contributing to comprehensive monitoring of patient outcomes and increased safety in medicine administration. The web application for accessing the protocols can be used independently of G8 pharmacy applications, while the necessary foundation for integrating G8 solutions with the PILK system is in place.
